Insurance

Elevate Medical is an in-network provider with Blue Cross Blue Shield

We are in-network with Blue Cross Blue Shield (BCBS) and soon with Medicare. Additionally, we accept auto accident, workers' compensation, and personal injury claims. Given the diverse range of coverage options, we encourage you to contact us to confirm if your specific plan is accepted. Our staff will verify and explain your benefits before treatment at no additional cost. For patients with out-of-network insurance providers, we will provide a superbill for your convenience.

Self Pay/No Insurance

We accept cash, credit cards and offer flexible payment plans.

If you have no health insurance or your insurance does not cover our services, we can work with you to create a payment plan so that you can receive the care you need.
